Take 5/HAZOB pocket books

A risk assessment must be undertaken before starting any task.  This assessment, also known as a Take 5 or Real Time Hazard Analysis (RTHA), ensures that you stop and think about what you are going to do before you commence work. 

An effective risk assessment tool will include a series of targeted questions to help you to:

• identify potential hazards that could impact you, your workmates, the environment and/or equipment while you are performing your work
• assess the hazards to determine the level of control that is required
• apply controls to manage the risk.
